The CMS assigned an overall rating of 1 out of 5 to Avir at Weatherford, falling short of the 2.7 average for Texas nursing homes.
Hilltop Park Rehabilitation and Care Center had the highest capacity within Parker County for the quarter, with 132 beds, while Willow Park Rehabilitation and Care Center followed with 125 beds.
As of the first quarter of 2026, Avir at Weatherford incurred $34,394 in fines across two incidents and faced two penalties.
